About Welcome Baby: Road to Resilience
Welcome Baby: R2R is a free, voluntary program that supports families in Yolo County during pregnancy and after birth. Families are referred by a healthcare provider or community organization, or may also sign up themselves.
Welcome Baby: R2R serves as a central access point for families. A dedicated public health nurse conducts an initial assessment to understand each family’s needs and uses that information to triage and coordinate appropriate referrals to home visiting, healthcare, and community resources.
Families receive a nurse visit after birth, and ongoing support from clinicians and parent coaches beginning in pregnancy and continuing through early childhood. Services can take place in the home or another location chosen by the family.
Families who join Welcome Baby: R2R may receive:

- Are pregnant or have a child under 2 years of age
- Live in Yolo County
