First 5 Yolo and Medi-Cal
First 5 Yolo is a Medi-Cal Community Health Worker (CHW) and Enhanced Care Management (ECM) Provider for Partnership HealthPlan of California Medi-Cal members. Partnership Medi-Cal members can receive these Medi-Cal services through First 5 Yolo’s Welcome Baby: Road to Resilience program and First 5 Yolo Lead Care Managers, available at no-cost to members. Welcome Baby: Road to Resilience helps build family resiliency and provides comprehensive support after the birth of a new baby. Learn more about the Welcome Baby: Road to Resilience program here.
Trained and experienced Community Health Workers and Lead Care Managers provide CHW and ECM services free of charge.
- If you are eligible to receive CHW benefits, a Community Health Worker will provide services that may include information, assessments, support, referrals, resources, and health education to address your health-related social needs.
- If you are eligible to enroll in ECM, a Lead Care Manager will provide comprehensive care management and coordination services that focus on clinical and non-clinical aspects of care to improve your health
The services that you will receive will help educate and promote positive parent and child physical, mental and emotional health and connect your family to additional resources that empower you with the tools and connections to thrive.
