Community Impact
First 5 Yolo leads efforts to champion prevention and early intervention for children and families, to build partnerships to improve systems of care, and to invest in proven programs and services. Since the inception of Proposition 10 in 1998, over $39 million has been invested in Yolo County with the understanding that the greatest opportunity to improve a child’s life occurs prenatally through the first five years of life.

First 5 Yolo’s 2024-2028 Strategic Plan is the road map to ensure that First 5 Yolo funds initiatives that provide a range of direct service programs and systems change efforts that benefit children ages 0-5 and their families. It was built with community input from parents, providers and stakeholders and is intended to address the unique child health and development, family support and systems change needs of Yolo County.

First 5 Yolo partnered with Start Early’s Early Learning Lab (ELL) to use human-centered design to help us learn how we might improve coordination and access to early childhood support services for all Yolo County children and families. F5Y led a project focused on improving “warm handoffs” practices and evaluation thanks to funding from the CalWORKs Home Visiting Initiative. First 5 Yolo worked with Health Communication Partners LLC and local families and providers on the co-creation of a knowledge product, “Warm Handoffs in Home Visiting Checklist.” Check it out here.
